Case Study: Population Health Management - Bim Group

THE CHALLENGE

Simply put, traditional wellness programs often fail. Its time employers start adopting a more holistic well-being and engagement model – one that not only identifies risk and the opportunity to reduce cost, but also aligns wellness incentives with behavior. TJ Regional Health gets it, and they teamed up with Bim Group to implement the Fit For Life Health & Wellness Center to help gain control over their cost of employee health care.

Here were their primary objectives:

  • Increase ease of access to Primary Care
  • Improve health outcomes among the health plan members
  • Remove cost as a barrier to obtaining recommended maintenance care for chronic conditions
  • Improve the focus on well-being and healthy lifestyles
  • Provide early diagnosis and interventions for chronic conditions
  • Accountability / face to face interaction
